How To Cancel An Appointment

Please use the following link or form to cancel appointments more than 24 hours in advance. If you are unable to attend an appointment, please let us know in plenty of time so that we can give your appointment to another patient.

The easiest and quickest way to manage your appointments and request repeat prescriptions is by using the Patient Access online service. Simply log in and select an option.  To be able to do this, you will need to register online. Please ask one of the Patients Services Team for an online registration form.

In a hurry?

If you are not registered for online services (Patient Access), or you don't have a password to hand, then you can complete the Cancel Appointment Form below:

If you need to reschedule your appointment, please contact the practice by phone or through Patient Triage. Please note that we may not be able to immediately rebook your appointment upon cancellation, as there may be a waiting period for the next available slot.
